I've read and happen to agree you should use your keyword to name pages. What if my keyword is 2 words? For example fishing rod, should I name it fishing rod.htm, fishingrod.htm or fishing-rod.htm??

first use fishing-rod.htm then fishing_rod.htm then if you still have more information, use fishingrod.htm
Never use fishing rod.htm it will only cause problems with search engines, links and browsers. You see the space ... they see %20 to replace the space.

So the search engines combine the words that are dashed into one keyword?

No, they view them as a keyword phrase ... keyword phrases are easier to rank well for than a single keyword. You should target your thinking to phrases ... to get optimum results for your work. If someone searches for dogfood as one word, then that is an obvious choice for a keyword, but remember ... it's easier to rank well with keyword phrases instead of just a keyword, so cover all the bases with quality content that lead into your primary objective.
How to feed your dogs healthy food .... leads into a page on types of dogfood you would recommend.
Feeding Your Dogs On A Schedule ... leads into types of dogfood again
Proper Dishes For Dog Feeding ... can lead into dishes they need to feed their hungry mutts!
All relative to dogfood, your primary focus, but with content the search engines love and keyword phrases that offer variation in your theme keyword.

Just so that you will know, Google does NOT use the UNDERSCORE (_) as a delimiter, so of the examples you gave, only ...
fishing-rod.htm
... Will be given ranking consideration for the keyword phrase ...
fishing rod
... And the keywords ...
fishing
... And ...
rod
This file name ...
fishing_rod.htm
... Will only be given ranking consideration for the query ...
fishing_rod
... And this file name ...
fishingrod.htm
... Will only be given ranking consideration for the query ...
