I've encountered merchants who express interest in an affiliate relationship but have no affiliate agreement. Attorney fees to create one may be prohibitive. Has anyone located an Affiliate Agreement form or template which can be used? Any other suggestions?

I think if the merchant is not ready for a full blown program, you should go and create your own "co-marketing" agreement. You should be able to set it up to be be only a few pages. The first section is for definitions, next for responsibilities of each party, then finally contractual boilerplate.
I would hope you could make it far simpler than the agreements we use, but you are free to "lift" content from my agreements:
