Hello,
a large company contacted me to run a 2-3 months campaign on one of my websites. They asked me send the advertising rate card. This website averages around 80,000 page views/month.
The ads rate card I'm preparing is below (Rates are for static ads only); in the table you see:
Type - Dimensions - Max File Size - CPM(in USD)

Super Scraper - 160 x 600 - 20K - $20
Banner - 450 x 90 - 20K - $15
Skyscraper - 160 x 400 - 20K - $15
Vertical Banner - 160 x 240 - 15K - $10
Vertical Banner1 - 120 x 240 - 20K - $7
Button - 120 x 90 - 10K - $4 -
Small Button - 120 x 60 - 10K - $3

Please, could anybody advise me on these rates? (or suggesting me a link where I can found how setting a ads rate card?). As I told above, this is for static ads, how much should I add for Flash ads?
Also, what does mean: "Rates posted are run of site positioning excluding Truck Buck page" ?

Ad rates vary enormously depending on the quality of the website and the niche you're in. See if you can find ad rates in your niche.

Run of site means appearing on every page.

I agree with Allan. There are many factors which would influence your ad rates. The amount and kind of traffic that your site has will be one of them. Another one would be your PR. But your rates seem reasonable enough Wink Are those rates for a monthly basis?
