You didn't specifically say what modifications you have seen that you are questioning. But here are some I have seen, and some I have done.
You can change the colors for the link title, description, advertiser url, background, and border to anything you like.
The sizes available include 728x90 leaderboard 4 ads, 468x60 banner 2 ads, 120x600 skyscraper 4 ads, and a 300x250 rectangle 4 ads.
Now, Adsense is javascript based, and you cannot alter the code provided by Google at all, you must paste it in "as provided," however, the ability to change things as I noted above is all within the Adsense control account you use.
One thing Google will not allow you to modify is the "Ads by Google" tag they put within the javascript. Google sets the color for this part automatically based upon the border color you set for your ads.
