I have seen some hosts have a setup fee. Have I to pay it always or can I do the setup myself and save the fee?

You cannot do the setup yourself. Setup means creating your user account, domain name database entries, and other functions that require administrator access to the web hosting companies computers. Even if you know how to setup accounts, no web hosting company will give you administrator access to their computers.
