I've had a difficult time managing a 1&1 managed server for a client of mine.
If you want a good, cheap dedicated server, I recommend ServerMatrix (servermatrix.com).
They're a subsidiary of ThePlanet, which (IMO) is one of the best dedicated server companies around. They have a great network and I've heard only good marks about them.
And yes, a dedicated server means that you get the entire server, and you are the only customer that has access to it.

EV1Servers
CartikaHosting
With a dedicated server it is important to go with a company that will provide a reliable service. Going with a cheap dedicated server could cause problems down the road. If you are using an unmanaged dedicated server it will be helpful to have a good System Administrator to watch over your server.
